HOW TO ENTER

Deadline for entries is 1 June 2010

The Shropshire Care Awards are open to all members of Shropshire Partners in Care and their employees.

Nominations are encouraged from those receiving care, their families, care professionals and organisations involved in providing care services, including members of SPIC. Remember entries are free for SPIC members.

Choose the category or categories for which you want to nominate your colleague or yourself. If you are entering more than one category, please submit separate entry forms. Nominations in each category will be limited to a maximum of 3 entries per organisation.

As part of the entry procedure, write an account of no more than 150 words saying why you think your colleague (or you) should be considered for an award.

All entries must be endorsed by the employee’s manager or a company director.

Supporting evidence will be required from the finalists in the form of statements from colleagues, residents or relatives which will be requested as part of the process to select the winners.
