The Care Awards are open to all members of Shropshire Partners in Care and their employees. Nominations are encouraged from those receiving care, their families, care professionals and organisations involved in providing care services, including members of SPIC. Remember entries are free for SPIC members.

Choose the category or categories for which you want to nominate your colleague or yourself. If you are entering more than one category, please submit separate entry forms.

Write an account of no more than 250 words saying why you think your colleague (or you) should be considered for an award. Supporting evidence from colleagues, residents or relatives are helpful but not essential.
